for use upon sample submission. For all of our clients and especially our
Pharmaceutical and biotechnology clientele, where FDA regulations are an issue,
we recommend filling out and signing the chain
of custody/order information form and sending with your sample. This form
provides sample tracking, sample preservation, clients address and contact
information and other pertinent information regarding the client and sample
and analysis to be performed. For our clients that are requesting a custom
formulation or deformulation of a product that involved divulging of proprietary
information, we recommend signing and sending along the non-disclosure agreement
form.
